Resumo: Introdução: Entender a participação e o real papel das protocaderinas (PCDH) e fundamental para melhor compreensão da carcinogênese, principalmente da carcinogênese oral e, possivelmente, identificar novos marcadores preditivos para Displasia Epitelial Oral (DEO) e Carcinoma de Células Escamosas Oral (CCEO), proporcionando o melhor entendimento do curso clínico dessas lesões. Até o momento, não existem trabalhos que tenham realizado investigação semelhante. Objetivo: A presente tese apresenta dois capítulos, cujos objetivos principais são: sumarizar a evidência científica sobre o perfil de metilação das PCDH na carcinogênese de neoplasias malignas humanas (capítulo 1) e investigar o perfil imunomolecular de PCDH10 na carcinogenese oral (capítulo 2). Métodos: No capítulo 1, uma Revisão Sistemática (RS) com meta-análise foi conduzida de acordo com a lista de verificação do PRISMA e a busca foi realizada nas bases de dados LILACS, PubMed, Science Direct, Scopus e Web of Science, de forma manual e sem restrição de data ou idioma. No capítulo 2, estudo imunomolecular foi realizado em CCEO, DEO e Mucosa Oral Normal (MON). Análise imunohistoquimica com anticorpos para PCDH10, 5-metilcitosina (5-mC) e 5- hidroximetilcitosina (5-hmC) foi realizada de forma quantitativa, com contagem do número de células imunomarcadas e em seguida foram categorizados pelo escore do percentual de células imunomarcadas (0 a 3), e qualitativa por meio de avaliação subjetiva com base na intensidade de marcação (0 = ausente; 1 = leve; 2 = moderado; 3 = intenso). Além disso, foi realizada análise da sublocalização celular da imunoexpressão de PCDH10 e foi obtido histoescore, no qual os níveis de imunoexpressão foram categorizados como negativa (0), baixa (1 a 4) e alta (5 a 12). A análise de metilação de PCDH10 foi realizada utilizando DNA genômico digerido (HpaII e MspI) e não digerido amplificado no sistema de reação em cadeia da polimerase em tempo real (RT-PCR) em casos de CCEO (n=7), DEO (n=5) e MON (n= 8). Resultados: Na RS, foram selecionados 41 artigos dos quais 26 foram utilizados na meta-análise. Observou-se que as PCDH mais estudadas na literatura foram: PCDH10, PCDH17 e PCDH8, sendo a associação entre metilação e os cânceres estudados de 26,08 (CI95% 15,42 – 44,13). No estudo imunomolecular, um total de 48 casos (18 de CCEO, 20 de DEO e 10 de MON) foram submetidos a análise imunohistoquímica. Observou-se perda de imunomarcação membranar de PCDH10 no CCEO; baixa imunoexpressão nuclear e citoplasmática em MON, DEO e CCEO (p<0,001). Evidenciou-se também maior imunopositividade para 5-mC em comparação ao 5- hmC, ocorrendo reducao na imunomarcação de MON para DEO e CCEO. A imunoexpressão de 5-mC foi alta (p<0,001), mostrando células marcadas no CCEO (p <0,001). Em contrapartida, para 5-hmC, houve maior imunomarcacão em DEO (p=0,001). O perfil de metilação de PCDH10 foi diferente entre os grupos, tendo as DEO maior status de metilação (p<0,001). Conclusão: A metilação de PCDHs e um importante evento epigenético na carcinogênese de diferentes neoplasias malignas e a metilação de PCDH10 pode atuar como mecanismo regulatório na carcinogênese oral. Ademais, pode-se inferir que a perda de 5-hmC pode ser um possível preditor de progressão em DEO.
Abstract: Introduction: Understanding the participation and real role of protocadherins (PCDH) is essential for a better understanding of carcinogenesis, especially oral carcinogenesis, and possibly to identify new predictive markers for Oral Epithelial Dysplasia (OED) and Oral Squamous Cell Carcinoma (OSCC), providing a better understanding of the clinical course of these injuries. So far, there are no works that have carried out similar research. Objective: This thesis has two chapters, whose main objectives are: to summarize the scientific evidence on the methylation profile of PCDHs in the carcinogenesis of human malignancies (chapter 1) and to investigate the immunomolecular profile of PCDH10 in oral carcinogenesis (chapter 2). Methods: In chapter 1, a Systematic Review (SR) with meta-analysis was conducted according to the PRISMA checklist and the search was performed in LILACS, PubMed, Science Direct, Scopus and Web of Science databases, from manually and without restriction of date or language. In chapter 2, an immunomolecular study was performed by means of immunohistochemical evaluation, with antibodies to PCDH10, 5-methylcytosine (5-mC) and 5-hydroxymethylcytosine (5-hmC) in OSCC, OED and Normal Oral Mucosa (NOM). The immunoexpression analysis was performed quantitatively, counting the number of immunomarked cells and then categorized by the score of the percentage of immunomarked cells (0 to 3), and qualitatively through subjective assessment based on staining intensity (0 = absent; 1 = mild; 2 = moderate; 3 = intense). A histoscore was obtained and immunoexpression levels were categorized as negative (0), low (1 to 4), and high (histoscore 5 to 12). The methylation analysis of PCDH10 was performed using digested (HpaII and MspI) and undigested genomic DNA amplified in the real-time polymerase chain reaction system (RTPCR) in cases of OSCC, OED and NOM. Results: In SR, 41 articles were selected, of which 26 were used in the meta-analysis. The most studied PCDH in the literature were: PCDH10, PCDH17 and PCDH8, with an association between methylation and studied cancers of 26.08 (95%CI 15.42 – 44.13). In the immunomolecular study, a total of 48 cases (18 of CCEO, 20 of DEO and 10 of MON) were submitted to immunohistochemical analysis. Loss of PCDH10 membrane marking was observed in OSCC; low nuclear and cytoplasmic immunoexpression in OSCC, OED and NOM (p<0.001). There was also greater immunopositivity for 5-mC compared to 5-hmC, with a reduction in the intensity of NOM labeling for OED and OSCC. The immunoexpression of 5-mC was high (p<0.001), showing cells marked in the OSCC (p<0.001). On the other hand, for 5-hmC, there was greater immunostaining in OED (p=0.001). The methylation profile of PCDH10 was different between groups, with the OED having a higher methylation status (p<0.001). Conclusion: PCDH methylation is an important epigenetic event in the carcinogenesis of different malignant neoplasms and PCDH10 methylation can act as a regulatory mechanism in oral carcinogenesis. Furthermore, it can be suggested that the loss of 5-hmC may be a predictor of progression in OED.
