Resumo: A doença renal crônica (DRC) constitui um importante problema de saúde pública que afeta principalmente os países ocidentais. No contexto da fisiopatologia dessa condição, o nível dos peptídeos natriuréticos, como peptídeo natriurético atrial (ANP) e uroguanilina (UGN), encontram-se elevados. Entretanto, não se sabe se os elevados níveis destes hormônios implicam em resposta natriurética adequada. Ademais, mostram-se necessários estudos que estabeleçam uma relação entre a DRC e a regulação de peptídeos natriuréticos e uma possível relação com a angiotensina II (AngII). Assim, buscou-se avaliar uma possível modulação da resposta à UGN pelo enalapril no modelo de nefrectomia 5/6 (nx 5/6). Utilizou-se ratos Wistar, machos, com peso entre 250-300g. Os animais foram divididos em 4 grupos (n=8): grupos controle sem tratamento ou tratado com enalapril (10mg/kg v.o.) (SHAM e SHAM+E) e grupos submetidos à nx 5/6 sem tratamento ou tratado com enalapril (10 mg/kg v.o.) (Nx e Nx+E). Ao final da 10ª semana após a cirurgia, os animais foram submetidos à técnica de microperfusão estacionária in vivo, no intuito de investigar o papel da UGN no túbulo proximal frente as diferentes condições. Posteriormente, foram determinados alguns marcadores de função renal e de estresse oxidativo, bem como quantificados os níveis de GMPc urinários. Nx apresentou os níveis séricos de creatinina, uréia, proteinúria, fração de excreção de sódio (FENa+) e malonaldeido (MDA) aumentados, e a taxa de filtração glomerular (TFG), além da atividade da superóxido dismutase renal (SOD) diminuídas. Nx+E, quando comparado a Nx, apresentou níveis reduzidos de creatinina, ureia, de proteinúria, da FENa+, e de MDA, além elevar a TFG. O grupo com DRC apresentou níveis baixos de GMPc na urina comparados ao controle, e o grupo tratado com enalapril atenuou em parte essa redução. Através da técnica de microperfusão estacionária, observou-se uma diminuição na reabsorção tubular proximal de bicarbonato nos animais submetidos a nx 5/6. A infusão intratubular de UGN não apresentou qualquer resposta nos animais com DRC, sugerindo insensibilidade do túbulo proximal a este hormônio nessa condição. Em contrapartida, nos animais do grupo Nx+E a UGN apresentou caráter inibitório sobre o fluxo de reabsorção de bicarbonato. Dessa forma, sugere-se que a modulação do SRAA na DRC aumenta sensibilidade do receptor GC-C à UGN a nível renal.
Abstract: Chronic kidney disease (CKD) is a major public health problem that affects mainly Western countries. In the context of the pathophysiology of this condition, the level of natriuretic peptides, such as atrial natriuretic peptChronic kidney disease (CKD) is a major public health problem that affects mainly Western countries. In the context of the pathophysiology of this condition, the level of natriuretic peptides, such as atrial natriuretic peptide (ANP) and uroguanylin (UGN), are elevated. However, it is not known whether high levels of these hormones imply in suitable natriuretic response. In addition, studies are required to establish a relationship between CKD and regulation of natriuretic peptides and a possible relationship with angiotensin II (AngII). Thus, we attempted to evaluate a possible modulation of the UGN response by enalapril in the 5/6 nephrectomy model (nx 5/6). Male Wistar rats weighing 250-300g were used. The animals were divided in 4 groups (n = 8): control groups without treatment or treated with enalapril (10mg / kg) (SHAM and SHAM + E) and groups submitted to nx 5/6 without treatment or treated with enalapril (10 mg / kg vol) (Nx and Nx + E). At the end of the 10th week after surgery, the animals were submitted to stationary microperfusion technique in vivo, in order to investigate the role of UGN in the proximal tubule in face of the different conditions. Subsequently, some markers of renal function and oxidative stress were determined, as well as urinary GMPc levels. Nx presented increased serum creatinine, urea, proteinuria, sodium excretion fraction (FENa+) and malonaldehyde (MDA), and glomerular filtration rate (GFR), as well as decreased superoxide dismutase activity (SOD). Nx + E, when compared to Nx, presented reduced levels of creatinine, urea, proteinuria, FENa +, and MDA, in addition to elevating GFR. The CKD group had low urinary cGMP levels compared to controls, and the enalapril group partially attenuated this reduction. Through the stationary microperfusion technique, a decrease in proximal tubular reabsorption of bicarbonate was observed in animals submitted to nx 5/6. Intratubular infusion of UGN did not show any response in the animals with CKD, suggesting insensitivity of the proximal tubule to this hormone in this condition. On the other hand, in the Nx + E group the UGN presented inhibitory character on the bicarbonate reabsorption flow. Thus, it is suggested that the modulation of the RAAS in DRC increase the sensitivity of the GC-C receptor to the UGN at the renal level.
